Forms For Athletes

The following forms are to be printed, completed, and reviewed by the athlete before  bringing them to the Head Athletic Trainer or Assistant Athletic Trainer for approval.  All athletes must have these forms completed before they can begin practice for the athletic season.

Freshman, Transfer Students

 IMPORTANT
All athletes must have a physical completed before their freshmen and junior seasons and turned in to the Athletic Training Office before they can participate.  Freshman and Transfer athletes will need to receive a physical by their primary health care provider and turn into Arnold Health Services .  IWU returning juniors can contact the Head or Assistant Athletic Trainer to set up an appointment to receive their physical at Arnold Health Services.
